All citizens of our country, including the smallest, have the right for medical care. Under the law the parents of the child can choose any pleasant treatment and prevention facility. If you move to the new residence or for some reason decided to replace one policlinic with another, it is necessary to attach the child to new medical institution.
Instruction
1. Do not take any actions if your child was only born. Newborns are automatically attached to policlinic at the place of residence. At discharge from maternity hospital the doctors will take an interest to what address the child will live. They will transfer information to the policlinic serving your house and already next day after an extract you will be visited by the district doctor and the nurse.
2. Take away the medical and inoculative record of the child from old policlinic if you change the residence. Notify the local pediatrician on moving, report the new address in registry and undersign for the magazine that medical documents of the child are received. Carry them in new policlinic and give to the attending physician. Will assign to the medical record number, it also will mean that now responsibility for health of your child lies on this medical institution.
3. Choose policlinic in which you want to observe the child if the policlinic at the place of residence does not suit you. Take away the out-patient and inoculative card of the child in registry of former medical institution. Append the signature in the special magazine which is in registry, that medical documents of the child at you on hands. Address to registry of the chosen policlinic and report that you want to attach the child for further observation. To you will suggest to write the application addressed to the chief physician then will number the card and will appoint the attending physician.
