The birth certificate of the child is the major document without which after majority it is impossible to receive the passport. Quite often there are situations when the certificate is lost, and it needs to be restored. What should I do? Where to address?
Instruction
1. Come to a regional REGISTRY OFFICE in which you registered the child (where received the birth certificate of the child). If you live at the moment in other city and to get to that REGISTRY OFFICE there is no opportunity, go to the REGISTRY OFFICE of that area where you live now.
2. Write the application of the corresponding form in that REGISTRY OFFICE where received the certificate. Attach passports of dad and mom, the certificate on marriage to it. The receipt on payment of the established state fee for recovery of the lost documents. The duplicate of the birth certificate is made out in one day.
3. Write the application in the REGISTRY OFFICE of that area where now you live if you have no opportunity to visit the REGISTRY OFFICE in which received the certificate. In this case the REGISTRY OFFICE will send the corresponding inquiry by the place of residence there where you received the certificate, on granting to you the duplicate of the lost document. This procedure will take slightly more time.
4. Appeal to court with the filled-in application form of the established form if it is impossible to claim the birth certificate from the REGISTRY OFFICE. The application is considered by court only in the presence of the answer of the REGISTRY OFFICE in writing about impossibility of providing the duplicate of the lost certificate to you.
5. Send the statement to the REGISTRY OFFICE by mail if there is no opportunity to visit body for the place of issue of the birth certificate personally. Copies of passports of parents, marriage certficates and receipts on payment of the corresponding state fee are put to the statement. The duplicate of the document lost by you is also sent by mail to the specified address.
6. Write the application in the REGISTRY OFFICE for repeated obtaining the birth certificate if you deal with this issue personally, and parents cannot help you (for example, are deprived of the parental rights, are divorced, etc.). The REGISTRY OFFICE will refuse to you on the basis of the fact that in this situation it is not possible to identify the applicant. You appeal against this refusal in court. In court your personality is established. After the introduction in the validity of the judgment, the REGISTRY OFFICE will issue you the duplicate of the certificate.
