1. Begin drawing up a response with the message of biographical particulars of the one whom you characterize. Enter the name and a surname of your child and also year of its birth.
2. If your kid attends kindergarten, write number or the name of preschool institution. Specify also age from which the child began to visit preschool institution. Describe the period of adaptation to a new stage of its course of life: whether quickly the child could get used to unfamiliar collective, accept and remember rules of conduct among other pupils of kindergarten.
3. Write what skills and abilities your child already seized: whether he is able to read or only to put syllables whether there are no defects of the speech, he is how interested solves logical tasks whether is able to put and read, etc.
4. Report about how long your kid can independently be engaged: to play or consider pictures in the book, to draw or to make something.
5. Write about hobbies. For example, the child very much likes to take dancing classes and is engaged in a circle of ballroom or national dances.
6. Open its relation to sport: whether it is engaged in any sports section that from sports classes (run, gymnastic exercises, swimming, figure skating, etc.) is pleasant to it most of all.
7. Write, your kid is how physically developed whether he has deviations in development according to age norms, whether there are no chronic diseases how often has catarrhal diseases.
8. Describe ability of your child to find contact with other children: whether he is always friendly or capable to show irritability and aggression whether he is ready to share the toys, whether he is able to carry out during some time one common cause together with other children.
9. Report about that, what does he think of weak, aged people, younger children. For example, you can write that he is always ready to come to the rescue and protect, to regret the offended, crying person. Means in it there are such traits of character as responsiveness, kindness, compassion.
10. Write whether your child loves animals, whether is able to care for them, whether he is capable to pass by a lonely and hungry kitten indifferently.
11. Describe how there takes place his communication with members of your family: whether he is tender whether he is balanced, capable to listen quietly to remarks and to correct the errors. Report about what instructions on the house he carries out.
