1. The school camp works 5 days a week within June and sometimes July. In August the school camps do not happen any more as all teachers are sent on leave. 2. Both mother, and the child's father can submit the passport of the applicant and his photocopy. In the copy of the passport it will be necessary to make the page with registration as the organization accepting the child needs to know where he lives and whom to address as a last resort. If the child lives not in a place of registration, it needs to be specified separately.
3. Birth certificate of the child. One more official document without which any procedure of registration to the camp including school will not do. Make copies from certificates of all children in family which are made out to the camp.
4. The certificate from work in F9 form of the one as whom the parent and in what company works. If one of parents does not work, another can file the statement and all documents or not submit this reference in general.
5. The reference from policlinic that the child is healthy, and in family is not present a quarantine. It is necessary to receive the reference right in front of the camp. At school of children the nurse before arrival has to examine in addition.
6. The statement of the parent with a request to accept the child to the camp. It is written to time of submission of all documents. The application form to you will be provided at the class teacher or the head teacher.
