Receipt in kindergarten causes many disorders from parents and the kid. The child enters the new world in which it appears alone with people around without presence of parents. The period of adaptation to a garden demands from adults of great patience. It is heavy to kid to come off people close to it, to get used to tutors and other children. But all this is preceded by time of collecting documents. This procedure will occupy the most part of time therefore it is important to plan the actions in advance. Competently knowledge of documents, necessary for receipt in a garden, will allow to distribute the time.
Instruction
1. The most important and difficult document in registration is a medical record. It is made out in policlinic at the place of residence. With this card it will be required to pass all specialists doctors: ophthalmologist, stomatologist, ENT specialist, psychologist, surgeon, neurologist, cardiologist, orthopedist, children's gynecologist. After survey by experts the map is filled out by the pediatrician. The calla on a yaytseglist and dab on enterobioz will be required to make tests of urine (this test is made in a week prior to visit of a garden). Of end the card is certified the sign and seal by the manager of preschool office of children's policlinic. The medical record has to be provided in kindergarten at filing of application on receipt. It will be required to attach copies of the birth certificate and the health policy to the medical record.
2. Certificate of family composition. It confirms registration of the child in that region in which it goes to a garden. If did not manage to register the kid yet, then at receipt in kindergarten it will be required to register it.
3. Birth certificate of the child and his copy.
4. The copy of the passport of mom and the questionnaire in which short data on parents are specified (full name, date of birth, the place of work and a position, contact phone numbers).
5. The documents or their copies confirming the privileges which are available for family. It can be the reference from social security authorities of recognition of family needy, the certificate of mother having many children or the reference confirming other privileges.
6. Permit about the direction of the child in kindergarten. In some regions the direction is given to a garden, but not for hands to parents. After that the staff of kindergarten reports to parents about need to come to a garden for paperwork.
7. After granting all the documents described above the application with a request for reception of the child in kindergarten is written.
