3. Recede from above several centimeters and in the middle write the word "Characteristic" from a capital letter.
4. Now write personal data of the child: surname, name, middle name, date of birth. This information at submission of various references in social security authorities is especially necessary. If characteristic is formed for other purposes, this point can be lowered.
