- - divorce certificate;

Instruction
1. You should visit department of the civil registration (CR) of a place of registration of the birth or of the residence. There you will submit the application for change of data, according to the established form.
2. Specify the in the statement a surname, a name and a middle name, date of birth, the residence whether you are married currently. In the presence of children list their names, surnames and middle names. Also specify details of the acts of civil status having to you relation, and a surname which you chose.
3. Enclose the following documents to the application: the passport, the birth certificate, the divorce certificate, birth certificates of children, documents confirming the reason of change of a surname. If in the enclosed documents there are contradictions, explain them in writing.
4. Pay duties on change of a surname and on replacement of each of certificates to which changes will be made. Usually in department the REGISTRY OFFICE can be taken the receipt for payment in bank, and in some departments there are terminals for payment of the state fees.
5. Your application will be considered in department the REGISTRY OFFICE, and within a month to you reported about the decision. If refuse to you change of a surname, you can appeal against this decision in Management the REGISTRY OFFICE.
6. If the decision was positive, make registration of change of a surname within a month. If you do not make it, then the decision will be cancelled, and the repeated petition can be submitted only in a year.
7. In the passport the stamp testifying to need of its replacement in a month will be put down.
8. The department the REGISTRY OFFICE in which you will record change of a surname will distribute data in those departments in which there are civil registrations which are subject to replacement. After modification, new acts will send you by mail. Changes include in the birth certificate, the certificate on divorce and in birth certificates of children.
